Once you have made the decision to join, simply contact my office to request a membership package.  The forms you will receive will include:

• New Patient Registration Form
• Records Release Form
• Patient Agreement Contract
• Medicare Opt Out Agreement (if applicable)
• Payment Option / Authorization Form

All forms should be completed and returned to my office.  Once I have reviewed your information, my office will contact you to schedule your first appointment.
