MEMBERSHIP INFORMATION
COSTS
Each patient pays a monthly fee, based on their
age, which enables the practice to limit the number of patients and provide more
personalized services. Premier Personal Healthcare offers the FEE-FOR-CARE concierge model, which
means we collect only the monthly fee, and nothing else. This fee will cover all
of the Internal Medicine services provided by our office, including exams, EKGs
and lab draws.
**Auto payments directly from your debit/credit card or bank account are available. ENROLLMENT For those considering membership, I invite you to schedule
an informal interview with me to ensure that this style of medical care is a
good fit for you and your family. I would like to personally answer any
questions you may have before you make your decision. Please call or e-mail my
office to schedule a visit. Once you have made the decision to join, simply contact my
office to request a membership package. The forms you will receive will
